At Brookwater General Practice, we believe in more than just treating illness — we support you to stay well and manage your health over time. For eligible patients, we deliver personalised care plans and annual health assessments, designed to coordinate care and support long-term wellbeing.
These services are bulk billed for eligible patients.

What is a Care Plan?
A GP Management Plan (GPMP) is a personalised health plan developed between you and your GP. It outlines your current health concerns, medications, lifestyle goals, and any other care you may need.
Care plans are particularly helpful for patients living with:
- Diabetes
- Heart disease
- Asthma or COPD
- Arthritis or chronic pain
- Mental health conditions
- Osteoporosis
- Cancer or complex recovery
- Multiple long-term conditions
If you also require regular input from allied health professionals (like physiotherapy, dietetics or podiatry), your GP may create a Team Care Arrangement (TCA) as part of your plan.
What is an Annual Health Assessment?
We also offer Annual Health Assessments for patients in specific groups, including:
- Patients aged 75 years and older,we can do this in conjunction with your drivers license renewal
- Aboriginal and Torres Strait Islander people from age 15
- ADF members discharging from service where eligible
- People with intellectual disabilities
These appointments are longer and more comprehensive than a standard visit. They provide a full snapshot of your health, covering physical, mental, and social wellbeing.
They include:
- Blood pressure, heart rate, and weight review
- Medication review and updates
- Lifestyle and nutrition assessment
- Mental wellbeing and memory checks
- Risk factors and early warning signs
- Referral coordination where needed

Eligible patients can access these services with no out-of-pocket costs. This includes:
- Chronic disease care plans (GPMP + TCA)
- Annual 75+ health assessments
- Aboriginal and Torres Strait Islander health checks
- Health assessments for people with disability
