Late Arrival, Cancellation & Failure to Attend (FTA) Policy

At MyLife Medical Brookwater General Practice, we are committed to providing timely and quality care to all our patients. To ensure fairness and reduce wait times, we kindly ask all patients to follow the guidelines below regarding late arrivals, cancellations, and missed appointments.

Late Arrival Policy

If you arrive 50% or more past your scheduled appointment time, this will be considered a Failure to Attend (FTA). Unfortunately, you will not be seen, and you will need to reschedule your appointment. An FTA fee will apply.

Cancellation Policy

If you fail to attend your scheduled appointment without giving 4-hour notice, a charge will occur at the Practice Manager’s discretion. If you are late to your appointment by more than half of the appointment time, you may be asked to reschedule your appointment and issued an FTA fee

What to Do if You’re Running Late or Need to Cancel

We understand that unexpected things can happen. If you’re running late or need to cancel:

  • Call reception as soon as possible
  • In some cases, we may be able to offer a telehealth appointment as an alternative (if clinically appropriate)

FTA Fee

A fee applies for Failure to Attend or for cancelling with insufficient notice. FTA fee is issued at Manager/receptions discretion.
Fee Schedule for Missed Appointments:


Standard Appointment: $65

Longer Appointment/ New Patient Appointment: $110

Procedure or speciality appointments (Mirena): $150
